TERMS AND CONDITIONS
All orders should be placed directly to the Customer Service Department in Skokie, Illinois, by telephone, fax or mail. UreSil’s customer service line is (800) 538-7374 and is available on weekdays (excluding national holidays) between the hours of 8:00 AM and 5:00 PM CST. UreSil’s fax number is (847) 982-0106. SHIPMENT AND TERMS
Items are normally shipped via standard ground transportation, unless the customer requests otherwise at the time of order placement. Shipping charges are prepaid and added to the invoice. Shipments are F.O.B. shipping point. Terms are Net 30 days. All prices are subject to change without notice. All products are shipped with a minimum of six months of shelf life.
UreSil encourages standing orders. Standing orders are normally scheduled for shipment on a monthly basis. To arrange for a standing order, call our Customer Service Department on our (800) number, or contact your UreSil sales representative. This convenience provides you with scheduled shipments using a single purchase order number.
We warrant that at the time of manufacture, this product was prepared and tested in accordance with regulations specified by the U.S. Food and Drug Administration, and is true to label. UreSil products are manufactured under the highest quality standards and conditions, and use of the product is a factor beyond the control of UreSil, which may directly affect it, and the results obtained from it. The user assumes responsibility for any consequences that may arise pertaining to the final use of the product. The foregoing warranty is exclusive and in lieu of all other warranties whether written, oral or implied (including any warranties of merchantability or fitness for purpose). No representative of UreSil may change any of the foregoing, or has any authority to bind UreSil to any representation or warranty concerning these products.
As we are continuously improving our products, the products supplied may not agree in all details with the description and specifications shown in the catalog or price list. UreSil reserves the right to change or discontinue any product without notice.
Federal (U.S.A.) law restricts these devices to sale by or on the order of a physician.
Authorization to return products is required for all returns. Request for return of products must be made by telephone. Contact the UreSil Customer Service Department (1-800-538-7374) for a return authorization number. Each return request should include the reason, purchase order number, UreSil invoice number, catalog order number, quantity and lot number. All products must be returned in their original unopened, sterile packages, and will be subject to a 20% restocking charge. Returns are only accepted within 60 days of the original UreSil invoice date.
Products that have been opened, damaged, expired or tampered with are not returnable for credit. Products manufactured as “specials or custom” products are not returnable for credit.
